Are those thin brown threads coming from it? It looks like how my euphyllias used to die, the tissue slowly sloughs off and the skeleton is exposed. I would soak it in Lugol's solution a couple of times a day for 15 minutes. Off-topic: I have two favorites, similar in content, I also removed them from the sand; where the living tissue contacts the sand, it dies, and sometimes if the sand isn't completely clean and algae are growing, they can also start to grow on the coral, thus killing it.

The "threads" that the corals started to release are a shedding of excess zooxanthellae after changing the light - a normal reaction. Skeleton exposure - trauma or bacterial infection, you did everything right, now you just need to determine a place where the coral will be comfortable, and then just feed it 1-3 times a week, if the parameters are in order.
