I have the same issue, sometimes once a month, spots appear on the body and fins. At first, I panicked and thought about treating it, but after asking around, I left everything as it was. After a few days, it all went away. If the aquarium has a healthy environment, it will resolve on its own. However, if it affects the gills, it can be serious. A sterilizer wouldn't hurt.

Today, the spots on the tail have disappeared, and there are traces on the dorsal fin. Overall, it feels pretty good. It eats algae and nibbles on the caulerpa.

I sometimes have a Navarchus that shows spots. It comes and goes from time to time in different places. It is a strong fish. However, it lives with zebrasomas, and so far, thank goodness, they are fine, with no spots.
